Accessing Past Immunization Records

Adults who need a copy of their past immunization records may be able to access them through the Texas Immunization Registry (ImmTrac2) if they previously consented to have their records stored in the system. To check if your records are available or to access them, visit the Texas Department of State Health Services ImmTrac website. If you did not sign up for ImmTrac2, your records will no longer be available through the state registry once you turn 18.

Submitting a request to retrieve archived records from the school district. 

To request a search for immunization records by the Victoria Independent School District, you must complete a "Transcript Request Form" in person at the VISD Administration building at 102 Profit Drive, Victoria, TX, 77901. There is an associated, nonrefundable, $5.00 cash payment due at the time of request for the search. Immunization records are not a required document to be retained by a school district, and therefore we cannot guarantee that the information can be produced.
